TOPIC: Question on OMB Requirements

post ko lng po tapos isasama ko na rin po yung mga tanong ko.

FOR ALL APPLICANTS
- Letter of Intent addressed to the Chairman -> sino pong chairman? barangay chairman o omb chairman?
- Duly filled-up and notarized application form -> san nakukuha yun application form?
- Four (4) pieces latest (2x2) signed photo of the applicant -> san po ididikit yung picture, sa application form po ba?
- Picture of establishment’s façade with permanent signboard
- Comprehensive location sketch of establishment
- Floor lay-out of establishments
- Latest ITR with stamp receipt of BIR
- Certification of NO Pending Administrative Case from BIR -> awts may bayad pa ba ito kapag kukuha ng certification?
- Two (2) pcs. proof of billing -> anu to? resibo?
- Two (2) pcs. official identification(Passport, driver’s license, SSS/GSIS ID)
- TCT or Lease agreement, if land is not owned by applicant
- Latest Financial Statement -> lhat ba ng statements income, balance sheet, cash flows?
- TIN of business of establishment
- Special Power of Attorney -> what the? lol? anu yan may bayad ba to??
FOR SINGLE PROPRIETORSHIPS
- NBI Clearance
- Bank Statements (not more than 15 days) -> anu to? financial statements????
- Certificate of Business Name from DTI

under single proprietorship, WTF na magkakaroon ng \"Table of organization and list of officers with addresses\"?!!!! Eh singol nga eh!!! WAAAAA! Maloloka ako....

ngayon ko lang din nakita yung mga requirements na iyan at mukhang overkill nga. but if you will notice on the actual form, there is a box before each item so i would presume that they will check only those required for a specific enterprise.

anyway, our ICA here in marikina will work-out a scheme with OMB wherein they will allow the association to accept and process the application of our members. we will let you know how it will work.

may question lang po ako tungkol sa omb. i think the raid is being done by the omb in coordination with the pnp and nbi. this is what we call as the anti piracy team ng pilipinas

tanong lang ho..... may mandate ba ho silang manghuli ng mga lumalabag sa piracy laws kung ito naka install na sa computer at walang makitang optical disk sa loob ng shop.... kasi po binasa ko yung batas na gumawa sa omb and andoon lang po ay tungkol sa mga optical disk wala po yung naka install na sa mga computers natin....

hindi ho kaya ang tamang office na dapat magpatupad ng mga raid ay ang intellectual property office o IPO na may hawak talaga sa mga copyrights at patents hindi ang omb dahil wala talaga sa mandate nila ito.

sana po malinawan tayo dito

alam mo, tama ka dun. kasi ang alam ko din, \"BSA\" ang dapat manghuli tungkol sa softwares, not OMB. Either BSA is with them during raids, or OMB personnel are overstepping their bounds?

Pero nabago na yung batas regarding this kasi dati individual warrants ang kailangan nila. ngayon yata, isa na lang, damay na lahat sa lugar na yun. So the law may have changed also. I'm also confused.<br><br>Post edited by: Bien, at: 2007/11/13 16:18

tito diego, ung proof of billing po ung mga billing notices na naka-address sa shop nyo like ung kuryente, tubig, isp, telephone.

Yes, our ICA has in its possession a document saying OMB can conduct raids on CD/DVD writers even without search warrants but raids concerning use of pirated software without the presence of PNP, NBI or BSA and a search warrant is still vague to us.

the BSA is just an organization of Software companies, yung OMB eh yung enforcement arm ng government regarding software piracy kaya yung OMB lang ang kinakailangan na present sa raids.<br><br>Post edited by: manoiskee, at: 2007/11/13 18:50
